Atopic dermatitis

Patient comes in for recheck on his complaints of allergic rhinitis and allergic conjunctivitis. He also has a history of atopic dermatitis. He currently uses Flonase one spray each nostril daily as well as Pazeo eye drops each eye daily as needed when he has acute allergic reaction. He also has triamcinolone ointment 0.1% that he uses for any atopic dermatitis lesions. He states that he has been doing very well with his allergy and has been well controlled just using Flonase daily to help with any congestion. He does have a cat allergy although he does not have cats. He does complain of some allergies that occur throughout the winter as well. His worse season is in the spring and summer with his grass and tree allergies and we talked about how he can provide the best relief for him using the medications and beginning them prior to blooming of tree season. He also can use Zyrtec 10 mg daily as needed. He does live in Caldwell so it is hard for him to come in and pursue allergy immunotherapy, although we will consider that if he continues to have problems with control of his allergies in the spring and summertime this year. His atopic eczema is very well controlled today. He states that he uses good moisturizer Nivea lotion every day after he showers and that has helped resolved some of his itching. He does not need a lot of the steroid cream at this time and his skin looks well today. He also has history of almond allergy. He has been avoiding almonds and I told him possibly next time we can evaluate his almond allergy and see if he does need to continue avoiding that. Otherwise, Gabriel is doing very well and he has no complaints today.

ASSESSMENT:
1. Allergic rhinitis.
2. Allergic conjunctivitis.
3. Atopic dermatitis.
4. Almond allergy.

PLAN:
1. At this time, we will continue Flonase one spray each nostril daily. I encouraged him to continue to do this through the winter as well since he is looking particularly allergic in his nose today.
2. Also he can use Pazeo eye drops for his eye complaints as needed.
3. He can also use Zyrtec for any breakthrough allergy symptoms as well as itching associating with his atopic dermatitis. He also has some triamcinolone 1% ointment for any outbreak of his eczema. I encouraged him a good skin hydration plan and reevaluated his practices to ensure that he has the best technique to combat his dermatitis.
